How do I enroll in one of the plans?

What is the name of the Insurance carrier?

The carrier is based on the chosen plan: Advent of Lloyds or Underwriters of Lloyds.

Can I apply for one week, one month, six months, or a year coverage?

A. Student Express, Student Liaison, or Inbound Immigrant plan: you can choose the time period from 1 day to 364 days. At the end of the time period you will receive an email indicating that your coverage will soon expire and you can enroll in a new plan.
B. Inbound Guest is available up to 180 days. At the end of the time period you will receive an email indicating that your coverage will soon expire and you can enroll in a new plan.

When will I receive my insurance Identification (ID) card?

Your insurance ID card and policy will be automatically sent to you via email. If you lose your ID card you can go back to your email and print a new card.

Is there a specific network of providers (doctors and hospitals)?

Yes, the provider network is Multiplan. You can view the network on our website.

My college requires the insurance company to complete a Waiver Request form and send to the college. Who do I contact to have the form completed?

You can call our office, 561-901-4900 or fax the form to our office, 561-908-2234.

Can I add my spouse and/or children to the plan?

Yes, the Student Liaison, Inbound Immigrant and Inbound Guest plans are available to add your family members.

Do I need to complete a Claim Form and where can I obtain the form?

At the end of the coverage period can I change to a new plan?

Yes, at the end of the coverage period you can change to a different plan.
